Water/Sewer Application
If you would like to sign up for water/sewer service, please complete and sign the appropriate form below. Once the form is completed it can be submitted by email ([email protected]), fax (570-662-3414), mail (14 South Main Street, Mansfield, PA 16933) or by dropping it off at our office or in the drop box outside of our office.
There is now a security deposit that is due on all new rental accounts. It is due at the time of application. Please call the office with any questions you may have.
